Are you a compassionate and detail-oriented LPN (Licensed Practical Nurse) looking to make a meaningful impact in home healthcare? Excel Home Care Services is seeking a dedicated LPN (Licensed Practical Nurse) to provide Medication Management to patients in their homes.
Position Overview:
As a Medication Management LPN (Licensed Practical Nurse) , you will play a crucial role in ensuring the safe and effective administration of medications for our clients. You will conduct in-home visits, educate patients and caregivers, and collaborate with healthcare professionals to enhance patient well-being.
Key Responsibilities of LPN (Licensed Practical Nurse) :

Conduct in-home assessments to review and manage client medications.

Educate clients and caregivers on proper medication usage , potential side effects, and adherence.

Set up and organize medications, including pre-filling medication planners when necessary.

Collaborate with physicians, pharmacists, and the care team to address medication-related concerns.

Monitor clients for adverse reactions and report any significant health changes.

Maintain accurate documentation of medication administration and client progress.

Qualifications:
