(LPN) Licensed Practical Nurse - 3 to 11

Join The Gardens at Country, a premier senior living community, as a Licensed Practical Nurse (LPN) on our team in Swansea, Massachusetts. As part of our healthcare team, you will play a vital role in delivering exceptional care to our residents, while upholding our values of compassion, dignity, and respect.

About the Role:

As an LPN at The Gardens at Country, you will be responsible for providing top-notch nursing care to our residents, with a focus on wound care, vital sign monitoring, and medication administration. You will work closely with our healthcare team to develop and implement individualized care plans, while maintaining accurate and detailed records.

Responsibilities:

  • Provide direct patient care, including wound care, vital sign monitoring, and medication administration

  • Develop and implement individualized care plans in collaboration with the healthcare team

  • Maintain accurate and detailed medical records, including charting and documentation

  • Monitor and report changes in patient condition to the healthcare team

  • Collaborate with other healthcare professionals to ensure quality patient care

  • Participate in shift reports and contribute to the overall success of the healthcare team
