Physical Therapist (Per Diem) – Putnam Hospital Center – Yorktown/Jefferson Valley, NY
Schedule: As needed, Monday–Friday, 8-hour shifts between 7:00 AM – 8:00 PM
Location: Yorktown–Jefferson Valley Professional Building, 2585 Navajo Street
Overview:
Putnam Hospital Center is seeking a Per Diem Physical Therapist (PT) to join our Physical Therapy Satellite Department. In this role, you will evaluate, treat, and educate patients of all ages—pediatric through geriatric—following a medically prescribed physical therapy plan. You’ll work with individuals recovering from injury, illness, or surgery to restore movement, reduce pain, and help them achieve optimal function and independence.
Responsibilities:
Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
Provide hands-on patient care focused on recovery, mobility, and long-term wellness.
Document patient progress and maintain accurate records in the electronic medical record (EMR) system.
Educate patients and families on exercises, safety, and rehabilitation goals.
Collaborate with physicians and interdisciplinary teams to ensure continuity of care.
Qualifications:
Required: Bachelor’s Degree in Physical Therapy from an accredited program.
Preferred: Master’s or Doctor of Physical Therapy (DPT) degree.
Preferred Experience:
1+ year of professional physical therapy experience.
Experience with EMR systems.
Background in orthopedics, neurology, or vestibular rehabilitation.
Skills: Excellent communication, critical thinking, and patient-centered care abilities.
Licensure: Active New York State Physical Therapist License or eligibility for a temporary permit.
Working Conditions:
Manual dexterity and fine motor coordination required.
Moderate to significant physical effort; potential exposure to typical clinical environment elements (e.g., odors, noise).
Compensation:
$67.00 per hour flat rate
