Full Time Benefits

Under the supervision of the PACE Clinical Manager, the Home Care Coordinator coordinates home care services (personal care and/or homemaking) schedule to meet participant needs. Serves as a liaison and assists Interdisciplinary team members to ensure participant needs related to home care services are implemented through coordination of care.

Responsibilities

  • Coordinate home-care services — Schedule and arrange personal care, homemaker, respite, and other in-home services based on participant needs.

  • Support individualized care plans — Ensure home-care services align with each participant’s care plan and changing needs.

  • Monitor service delivery — Track visits, attendance, missed services, and overall quality of care provided in the home.

  • Collaborate with the interdisciplinary team (IDT) — Communicate participant updates and concerns with nurses, social workers, providers, therapists, and other PACE team members.

  • Coordinate with caregivers and agencies — Serve as a key point of contact for home-care agencies, caregivers, participants, and families.

  • Address participant needs and concerns — Respond to scheduling issues, caregiver concerns, changes in condition, and potential safety risks, escalating when appropriate.

  • Maintain accurate documentation and compliance — Document services and communications in the medical record and ensure care coordination follows PACE policies and regulatory requirements.

Qualifications

Education

  • High School Diploma or GED

  • Completion of a nursing assistant training program totaling at least 75 hours or equivalent education, training and experience.

License(s)/Certification(s)

Maintains current CPR status or acquire within 30 days of hire. Maintains current Mandatory Reporter status or acquire within 30 days of hire. Completes 12 hours of related in-services per calendar year. Valid driver’s license when driving any vehicle for work-related reasons. Valid Auto insurance
