Description
Klarus Home Healthis looking for a full time / part time or PRNLicensed Practical Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receivinghome health services in the Englewood, Aurora, Lakewood and/or Denver metro area.
The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
Requirements
Key Responsibilities:

Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.

Coordinate care with the interdisciplinary group, patient/family, and referring agency.

Assist in the development and implementation of patient care plans.
-
Monitor patient health and report any changes to the supervising RN.
-
Administer medications and treatments as prescribed.
-
Educate patients and their families on health care needs, conditions, and options.

Maintain accurate and timely documentation of patient care and progress.
Job Qualifications:
-
Education:Graduate of an accredited School of Vocational/Practical Nursing.
-
Licensure:Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.

Experience:Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.

EMR system: HCHB experience highly preferred

Skills:Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.

Transportation:Reliable transportation and valid, current auto liability insurance.
Environmental and Working Conditions:

Work in patients' homes in various conditions.

Ability to work a flexible schedule and travel locally.

Ability to lift up to 50 lbs and move patients.
