Exciting opportunity to join the stable BJC Home Care team in Farmington, Missouri! Supporting a wide range of patient needs, including orthopedics and stroke recovery.
If you're a self-starter, excellent communicator, organized, and have top-notch time management skills, this could be the perfect fit for you. This position supports one Occupational Therapist.
Location:
Farmington (Primarily supporting Bonterre to Fredricktown, with flexibility for a larger area as need).
Schedule:
Monday through Friday.
No weekends, no on-call duties, and no holiday work required!
Overview
BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.
The Parkland Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Farmington, Missouri and several nearby counties, with 24 hour on-call home care nursing supervision. Our JCAHO accredited, multi-disciplinary approach combines leading edge technology with a firm belief in the powerful recuperative advantages of receiving home care.
Preferred Qualifications
Role Purpose
Provides skilled treatments to patients as prescribed by the physician and directed by a licensed occupational therapist as specified in their health care service setting.
Responsibilities

Implements treatment plan as developed by the licensed occupational therapist.

Collaborates with the licensed occupational therapist on an agreed upon frequency to assess and reassess patient’s plan of care, treatment goals and interventions.

Documents patient progress in daily notes and weekly notes (as applicable), as well as contributes recommendations to patient’s discharge note from occupational therapy

Reports on interventions and patient progress during assigned meetings and contributes to the screening and evaluation process.

Assists with guidance for clinical fieldwork students under the supervision of the licensed occupational therapist.
