Position Summary
We are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. In this role, you will provide high-quality, individualized therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. Flexible scheduling is available, with both full-time and per diem opportunities.
Key Responsibilities

Evaluate, plan, and implement individualized physical therapy programs for residents

Provide therapeutic interventions including mobility training, balance exercises, strengthening, and gait training

Monitor and document resident progress, updating care plans as appropriate

Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care

Educate residents and families on exercises, adaptive techniques, and safe mobility

Participate in team meetings and quality improvement initiatives

Maintain compliance with federal, state, and facility regulations
Qualifications

Licensed Physical Therapist in the state of Iowa

Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred (new graduates welcome)

Strong clinical reasoning, communication, and documentation skills

Ability to work independently and as part of a multidisciplinary team

CPR certification required or willingness to obtain
