JOB SUMMARY

The Occupational Therapist, Home Health, evaluates, plans, and administers comprehensive occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician.

RESPONSIBILITIES AND SCOPE

  • Manages an occupational therapy caseload with efficiency and efficacy

  • Completes clinical documentation in the medical record per department guidelines and completes department records for billing and statistics. Performs accurate assessments and documentation for evaluations, OASIS, discharge, and regular visits.

  • Manages resources needed to perform OT treatment plan to accomplish the goals set for the patient. Provides, modifies, or discontinues occupational therapy based upon an ongoing assessment/reassessment of the patient’s clinical status in accordance with physician orders

  • Selects and performs appropriate Occupational Therapy (OT) evaluation procedures to identify and assess problems. Sets treatment goals and devises a treatment plan. Re-evaluates and adjusts the treatment plan in response to the patient's changing condition and discharge plans.

QUALIFICATIONS

Required

  • Bachelor’s degree in Occupational Therapy required with curriculum approved by the American Occupational Therapy Association.

  • Current valid NC license in Occupational Therapy.

  • Minimum of two (2) years of work experience in Occupational Therapy. Prior Home Health experience preferred.

  • Basic Life Support (BLS) required.

Preferred

  • Prior Home Health experience preferred.
