As a Home Health Aide on our H ospice team , you will care for patients where they need you most, whether at home, a nursing facility , assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones.

A caregiver in this position works 8-hour shifts on an as-needed basis (Minimum 48 hours in a 6-week period).

A caregiver who excels in this role will:

  • Perform basic patient care activities.

  • Assist with procedures ordered by physician , with supervis ion from an RN, PT, OT or ST as appropriate .

  • Assist patients with tending to personal care, activities of daily living and transfers/transport.

  • Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.

  • Assist with keeping the patient's environment clean and orderly.

  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.

  • Escalate change in patient condition to patient’s physician as appropriate .

Minimum qualifications for the ideal future caregiver include:

  • High S chool D iploma or GED

  • State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry

  • One year of clinical experience as an A ide in an acute, rehab or skilled nursing setting

  • Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage

  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) within orientation period

Preferred qualifications for the ideal future caregiver include:

  • Hospice and Palliative Care Nurse Assistant (CHPNA) C ertification

  • Home C are experience ​

  • Available to attend a full-time three week new hire orientation preferred

Physical Requirements:

  • Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.

  • For field staff only: Able to drive to patients’ homes and/or facilities.

  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

  • Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
