Description: Position Purpose
At Saint Mary's Hospital our RN Case Manager are responsible for coordinating patient care, assessing needs, and developing care plans. They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care well advocating for the patients' well-being.
What you will do:
- As a Registered Nurse RN Case Manager Department, you will serve as a driver of a collaborative, process which coordinates, monitors, and plans health care services throughout the patients’ illness.
Minimum Qualifications:
Required:

Graduation from an accredited nursing school

Valid RN license in Connecticut
Preferred:
-
Bachelor's degree in Nursing
-
Five years of diversified, progressive experience in acute care and/or other settings within the continuum of Care including an understanding of nationally recognized medical utilization criteria, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities.

BLS certification required
