Part Time Regular Part-Time

Webster, NY, US

Salary Range: $22.00 To $24.00 Hourly

Summary:

Provides day-to-day oversight for Family Care Program, ISS Program, and Families In Need Program. Provides supervision for said programs to ensure compliance with all fiscal, regulatory, and CDS Monarch policies and procedures. Acts as a representative of CDS Monarch at all times for the benefit of all said programs.

Essential Job Functions:

  • Ensures daily program compliance with OPWDD regulations and CDS Monarch Policy and Procedures for all programs.

  • Ensures any re-certifications are submitted in a timely basis.

  • Assures all paperwork is completed and policies are adhered to.

  • Ensures required staff training compliance is met for all applicable programs.

  • Reviews monthly documentation as it pertains to each program.

  • Maintains organized records pertaining to each program.

  • Completes requests and submits for payments as applicable to each program.

  • Ensures compliance with all billing requirements.

  • Provides administrative on-call support for Family Care providers and individuals.

  • Attends all annual and bi-annual case reviews and creates/revises individuals protective oversight plans and residential habilitation plans. Follows agency policy and procedure for record storage and retention.

  • Provides designated quality assurance inspections of Family Care records and homes.

  • Completes Family Care Provider payroll.

  • Attends home visits monthly.

  • Reviews and obtains approval for Families in Need requests.

  • Completes and submits bi-annual ISS reports.

  • Performs all other necessary duties relevant to the position as requested by the supervisor.

Knowledge, Skills, and Abilities:

  • Knowledge of Medicaid Services Coordination and/or Broker.

  • Experience working with families and customer service.

  • Ability to work independently and motivate others.

  • Excellent communication skills.

  • Must be at least 18 years of age.

  • Maintain all require certification/training by State regulation and CDS Monarch policy.

Education and Experience:

  • Bachelor’s Degree in a human service related field preferred.

  • Two years’ experience in Service Coordination with individuals with Developmental Disabilities.

  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.

  • Attend mandatory education and training modules as scheduled; obtain and maintain required certifications.

  • Maintain all required certifications/training by State regulations and CDS policy

  • Act as a professional representative of CDS Life Transitions, Inc. in regard to appearance, behavior, temperament, communication, language, and dress.
