We are looking for a caregivers, certified home health aide and CNA to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role .

RESPONSIBILITIES: Must have exp. working with client with Asperger's syndrome, stroke patient and an Amputee, who is very active. Assist client to all medical appointments 2-3 times a week (will be compensated for travel), medication reminder Insulin before meals. The client like to go to the YMCA weekly for activities for 1 1/2 hours, and like walking his dog regularly. General companionship and community engagement support.

The preferred schedule is Monday through Thursday (11 am to 3 pm) Must have your own car and insurance.
