Description

The HARP/RCA Care Manager provides linkage to Home and Community Based Services (HCBS) for Clients who meet the Behavioral Health threshold identified by the state. The HARP/RCA Care Manager promotes linkage to needed HCBS through the completion of assessments, crisis plans, and Health Plan Summaries. The HARP/RCA Care Manager will engage the clients in a person-centered approach and complete ongoing collaboration with community partners, providers and supports systems to ensure best health care outcomes.

Duties

  • Responsible for, but not limited to outreach and engagement, assessment completion, service and treatment linkages and coordination using evidence-based practices and outcomes.

  • Comprehensive client centered Care planning development and management with the individual to assist in obtaining and maintaining HCBS.

  • Coordination of care with Finger Lakes Community Health Providers and FLACRA clinic providers as well as ongoing coordination of care with other providers identified in the care plan.

  • Build and use effective communications strategies among peers, medical staff, addiction and mental health providers and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology.

  • Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.

  • Participate actively in team meeting and supervision to enhance in professional development.

  • Maintain excellent record keeping.

  • Use of personal vehicle for work and potential transportation of clients

  • On-Call duties as assigned for nights and weekends.

  • Other duties as assigned.

  • Demonstrates ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.

  • Knowledge of local behavioral health services and substance abuse agencies.

  • Ability to multi-task in a fast-paced environment, have good problem-solving skills, as well as excellent time management and organizational skills and the ability to remain calm in a crisis while providing crisis intervention.

  • Must be able to type 30 WPM.

  • Proficient in Internet navigation, Microsoft Office, Outlook, Word, and Excel.

  • Proficient in the use of electronic health records and ability to utilize multiple platforms.

Qualifications

Minimum Qualifications:

A HARP Care Manager must meet a combination of the following qualifications:

Education

  • Bachelor’s Degree in any of the following: Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Physical Therapy, Recreation or Recreation Therapy, Counseling, Community Mental Health, Child and Family Studies, Sociology, Speech and Hearing or other Human Services Field OR

  • A NYS teacher´s certificate for which a Bachelor´s Degree is required OR

  • NYS licensure and registration as a Registered Nurse and a Bachelor´s Degree OR

  • A Bachelor´s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses OR

  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)

Experience

  • Two years’ experience (a Master’s degree in a related field may substitute for one year’s experience) either:

  • Providing direct services to persons with serious mental illness, developmental disabilities, or substance use disorders OR

  • Linking persons with serious mental illness, developmental disabilities, or substance use disorders to a broad range of services essential to successfully living in a community setting

Drivers licensed required.

Participation in On-Call Rotation nights/weekends.
