Job Description:

Schedule: Full-Time/Day Shift: Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation. Location: Forest park Maywood Berwyn North Riverside Forest View Westchester Area with travel to LaGrange Hospital for meetings/Orientation etc.

• Implements and monitors patient care plans to ensure effective treatment.

• Monitors, records, and communicates patient condition as appropriate.

• Serves as the primary coordinator of all disciplines for well-coordinated patient care.

• Notes and carries out physician and nursing orders accurately and promptly.

• Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.

• Administers medications and solutions as required.

• Documents all phases of patient status during care.

• Participates in the transfer of patient information to ensure continued quality care.

• Leads and mentors staff to ensure optimal performance and competency in care delivery.

• Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.

• Develops and maintains positive working relationships within the practice and other departments.

• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

• Bachelor's of Nursing [Preferred] Field of Study:

• 1+ acute care clinical experience [Preferred]

• 1+ home health experience [Preferred] Additional Information:

• Registered Nurse (RN) [Required]

• Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:

$33.60 - $56.52
