Pharmacy1st Health is building the clinical infrastructure for rural America by transforming independent pharmacies into integrated care delivery hubs. Our model combines primary care, pharmacy services, and chronic disease management under one roof, creating a new, lower-cost site of care for underserved communities. We operate across multiple states and are rapidly expanding a network of rural clinics co-located with pharmacies. Our goal is to deliver scalable, high-quality care while improving access, outcomes, and cost efficiency. Description From day one, you will play a central role in shaping how care is delivered in a community where access truly matters. You will practice with autonomy, purpose, and support while helping launch a modern, community based care model that blends primary care, prevention, and technology enabled access. You will also help influence the growth of the care team by participating in the recruitment and selection of future clinical roles, ensuring we build a team aligned in values, quality, and commitment to the community. What You Will Do

Deliver comprehensive patient care with a strong focus on prevention and continuity

Serve as a clinical leader and trusted partner to patients and families

Collaborate with pharmacists, care coordinators, and virtual care partners

Help design workflows, care standards, and patient experience from the ground up
-
Contribute ideas and leadership as the clinic scales and evolves

Refer and partner with specialists or community resources where appropriate for patient clinical conditions
Who You Are
-
A Family Nurse Practitioner with a Master’s or Doctoral degree in Nursing
-
Licensed in (Insert Location) with national certification such as FNP-BC or FNP-C
-
Experienced in primary care, community health, or rural health environments
-
Confident using EHR systems and telehealth platforms
-
Entrepreneurial, adaptable, and excited by building something meaningful

Deeply committed to patient centered, relationship driven care

Must be able to safely perform the essential functions of the position, including prolonged standing, walking, occasional lifting of up to 25 pounds, and assisting with patient care in a clinical environment, with or without reasonable accommodation.

Maintains all necessary state and CMS credentials for professional nursing practice

BLS Certified
