ECM Case Manager

Location: San Jose, CA

Position Type: Full-Time

Category: Healthcare

Salary: $25.00–$28.50 per hour

Position Overview

We are seeking a compassionate and organized ECM Case Manager to support high-risk Medi-Cal members with complex medical, behavioral health, and social needs. The Case Manager will provide outreach, engagement, care coordination, and connection to community resources to help clients achieve improved health, wellness, and stability.

This is a community-based position involving work with clients in homes, shelters, schools, community organizations, and other partner locations, as well as office-based work.

Key Responsibilities

  • Conduct outreach and enrollment activities for eligible members participating in the CalAIM Enhanced Care Management (ECM) program.

  • Complete behavioral health, housing, and other high-risk assessments to identify client needs.

  • Develop and maintain individualized care plans.

  • Coordinate services with multidisciplinary healthcare teams, providers, community organizations, and outside agencies.

  • Facilitate referrals, resource navigation, and access to healthcare, housing, wellness, and community services.

  • Help clients develop and strengthen social and community support systems.

  • Advocate for clients and establish effective working relationships with community agencies and service providers.

  • Maintain accurate and timely clinical documentation in accordance with applicable standards and regulations.

  • Provide services in diverse community settings, including shelters, residential facilities, drop-in centers, and client homes.

  • Participate in clinical supervision, consultation, training, and team meetings.

  • Provide translation or language support when appropriate.

  • Perform other related duties and projects as assigned.

Qualifications

  • Bachelor's degree in healthcare, behavioral health, social services, or a related field, or equivalent education, training, and experience .

  • At least 1 year of case management or related experience .

  • Familiarity with community-based resources, particularly housing and social services .

  • Strong problem-solving, organizational, communication, and interpersonal skills.

  • Understanding of cultural considerations, community resources, and family dynamics.

  • Trauma-informed care training preferred.

  • English fluency required.

  • Spanish, Mandarin, Vietnamese, or another second language is a plus.

  • Valid California driver's license and current personal auto insurance required.

  • Must meet applicable vehicle-insurability requirements.

Work Environment

This position requires regular community-based work and travel to client homes, schools, shelters, residential facilities, community organizations, and other partner locations. Driving is an essential function of the position.

Pay Details: $25.00 to $28.50 per hour
