City/State Charlottesville, VA Work Shift First (Days) Overview: Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN.
Position Status: Full Time - Day Shift
Position Location: Charlottesville, VA
Mileage reimbursement will be paid for all miles driven between patient visits.
We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits.
Overview The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. Includes interaction with other facility staff. Actively participates in interdisciplinary team meetings. Conducts initial and periodic assessments. Administer nursing care to ill, injured, convalescent or disabled participants. Advise participants on health maintenance and disease prevention. Administers medication if needed.
Education
- Associate's Degree Nursing (Required)
Certification/Licensure
- RN (Required) - Active in VA
- BLS (Required within 90 days)
Experience
- 1-year minimum nursing experience (required)
- Home Health experience (highly preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $87,214.40 - $126,900.80 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
