NURSE PRACTITIONER - HOSPICE AND PALLIATIVE CARE SERVICES

Responsibilities

  • Proactive, collaborative team member in a hospice care environment

  • Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care

  • Commitment to caring for patients and partners

  • Complete all documentation and records in a timely and accurate manner

  • Follow established policies and procedures as related to the scope of practice

  • Maintain continuity of patient care in a hospice setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

  • Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice

  • Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
