Team 2 Coverage Area: Grand Haven, Spring Lake, West Olive; or Team 4 Coverage Area: Coopersville, Nunica, Bailey, Ravenna

An innovative home care LPN uses cutting edge technology and clinical knowledge to provide exceptional care to patients who require one on one attention and monitoring in the scared place they call home. Compassionate care is provided to all members of our communities through a multidisciplinary approach working collaboratively with physicians, rehab team members, social workers, and home health aides. Minimum Qualifications

  • Graduate from an accredited or state approved school of practical nursing
  • Current licensure in the State of Michigan
  • CPR certification required.
  • Must have a minimum of one (1) year experience in an acute care setting.
  • Home healthcare experience preferred.
  • Must have current Driver’s license and reliable transportation to and from work site.
