Job Summary Performs biopsycho-social assessments, creates and updates individualized treatment plans, provides individual, family and group therapy for patients to Outpatient Behavioral Care Services. Specializes in work with adults, and/or adolescents, and families experiencing mental health/substance abuse problems.

Essential Functions

  • Completes an accurate biopsychosocial assessment, including competency in diagnostic determinations and determination of appropriate level of care for patient.

  • Provides direct and indirect clinical/therapeutic services to patients and their families with clinical supervision as needed.

  • Develops and individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment plan. -

  • Provides the most appropriate therapy, specific to patient's symptoms.

  • Performs any case management responsibilities involved with direct patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement/Education, timely and appropriate documentation of care provided, communication with referral sources, completion of short-term disability paperwork and communication with patient's employers as well as coordination of care with multidisciplinary treatment team and outside community providers.

  • Participates in outcome monitoring activities.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Master's degree in Social Work, Counseling, Psychology or related human services field of study.

  • Experience - No previous experience required

Required Certifications, Registrations, Licenses

  • Currently licensed as a Licensed Master Social Worker (LMSW) in the state the team member is working, or currently licensed as a Licensed Professional Counselor-Associate (LPC-A) in the state the team member is working, Marriage and Family Therapists and Psycho-Educational Specialists, or currently licensed as a Marriage and Family Therapist-Associate (LMFT-A) in the state the team member is working.

Knowledge, Skills and Abilities

  • Group, individual, and family therapy skills

  • Psychiatric problem-solving abilities, assessment skills, treatment planning skills.

  • Communication skills

  • Interpersonal skills across multiple professions/disciplines

  • Knowledge of computer software and utilization of electronic medical record.
