Full job description

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and Gurwin Certified and Licensed Home Care Services Agencies.

We are currently seeking a Per Diem Pharmacist who will embody the passion and commitment required to deliver the ultimate care for our residents. Must be available to work Saturdays 8:00am -4:00pm, Sundays 8:00am-12:00pm and other days as needed. The candidate will be responsible for caring for geriatric residents to ensure regulatory compliance and to continue our reputation of providing the gold standard of excellence in quality care for those we serve.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Pharm D preferred or BS in Pharmacy

  • Hospital or LTC experience required

  • BCGP (cerified geriatric pharmacist) preferred

  • Current New York State License to practice pharmacy.

DUTIES AND RESPONSIBILITIES:

  • Interprets and enters drug orders and patient data into computer profile.

  • Compounds IV’s and dispense medication upon written prescription from staff physicians, staff physicians’ assistants, and staff nurse practitioners.

  • Provides drug information to medical and nursing staff.

  • Supervises all functions of pharmacy technician staff.

  • Reviews resident’s medical drug regimen on a monthly basis, reporting any medication irregularities, adverse drug reactions, or possible problems related to multiple drug therapy for the geriatric and making recommendations.

  • Maintains computerized patient profiles for all residents.

  • Dispenses medication in a manner which meets all State and Federal prescription laws,

  • Demonstrates the ability to discern the medication needs and behaviors of the subacute/rehab population and to communicate to residents and/or family members relating to medication regimen.

  • Maintain records and inventory of medications.

  • Make recommendations to medical staff regarding formulary.
