Care Transition Manager RN Registered Nurse

Part-time

  • Work location: Texas Health Azle, 108 Denver Trail, Azle, TX 76020
  • Will cross-train/cover at Texas Health Alliance at 10864 Texas Health Trail, Ft. Worth, TX 76244
  • Care Transition Management department
  • Work hours: Part-time -24 hours/week, flexible coverage 7 days per week: Between Monday-Friday 7:30AM-4:00PM, on weekends Saturday & Sunday between 8:30AM-1:30PM, and ER coverage from 11:30AM-8:00PM.

  • (Does Require 4-weeks of training Monday to Friday 7:30AM-4:00PM)

Here's What You Need

  • Bachelor's degree in nursing (BSN) required. (Individuals hired as a Care Transition Supervisor prior to May 11, 2017 will be grandfathered with an ADN at the entity they were employed at on May 11, 2017)

  • 3 years staff nurse at an acute care hospital required

  • 1 year discharge planning/care management highly preferred

  • RN - Registered Nurse upon hire required

  • CPR - Cardiopulmonary Resuscitation upon hire required and

  • ACM - Accredited Case Manager upon hire preferred Or

  • CCM - Certified Case Manager upon hire preferred Or

  • ANCC upon hire preferred

What You Will Do

  • Working knowledge of medical necessity criteria preferred

  • Knowledge of Microsoft Outlook and Office (Word, Excel)

  • Customer service skills

  • Ability to engage in complex clinical decision-making and discharge planning

  • Strong oral and written communication skills

  • Strong commitment to interdisciplinary collaboration

  • Critical thinking, analysis and conflict resolution skills

  • Psychosocial and crisis intervention skills

  • Ability to prioritize and meet deadlines

  • Identify community resources and service needs and facilitate appropriate referrals as needed.
