Summary
This position serves as the Program Support Assistant for the Home Based Primary Care and Non-Institutional care programs. This position is responsible for the day-to-day management of the Home Based Primary Care (HBPC) admissions and discharge procedures, which provides the administrative foundation for the functioning of the HBPC program. This position is location in the Department of the Veterans Affairs, Veterans Health, Charles George VA Medical Center, Asheville, NC, (CGVAMC).
Duties
Major duties and responsibilities include, but are not limited to: Collect and validate information necessary for the appropriate PACT team assignment and enters the necessary data into a variety of databases for use at program, local, and national levels. Remove information from all of the appropriate databases and ensure the patient is properly assigned elsewhere upon patient discharge. Ensures the patient/family member receives written information regarding their status for any needed action. Ensures all processes are accurate, timely, and complete and initiates immediate follow up in response to any incomplete information or inconsistencies among documents. Responsible for the development and maintenance of a database which allows for compiling and tracking of data points necessary for the preparation of reports and the completion of numerous other functions, such as GIS mapping, identifying high priority patients who might need specific assistance in the event of a weather or human- caused emergency, identifying patients on oxygen or ventilation support, and listing of patients who are of Do Not Resuscitate (DNR) status. Serves in a backup role for the numerous programs which are coordinated out of the Non-Institutional Care. Work Schedule: Monday- Friday 8:00am-4:30pm Telework: This position may be authorized for telework. Telework eligibility will be discussed during the interview process. Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/07/2026. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Collects, validates, and analyzes data from a variety of sources; creates and maintains and modifies a variety of databases for program tracking and analysis; and serves as a coordinator for patient enrollment programs.. Your qualifications will be evaluated based on your application materials (e.g., resume, supporting documents), your responses on the application questionnaire, and your responses to all assessments required for this position. Applicants will be required to complete a Supervisory Situational Judgment Test (SSJT) as part of the assessment process for this position. The SSJT evaluates judgment and decision-making skills that align with relevant competencies, organizational values, and best practices. Assessment results may be used as part of the evaluation, ranking and referral process. This is not a timed test, however, the assessment is expected to take approximately 45 minutes or less to complete. The assessment for this application does not include a cut score for a minimum level of required proficiency in these critical general competencies. However, you must answer each item in the assessment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work required does not inherently include any physical requirements essential for successful job performance that could not otherwise be performed with accommodation or workplace adjustment. Receiving Service Credit for Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
