Home Health Scheduler
JOB SUMMARY:
The Home Care Scheduler is responsible to coordinate and maintain scheduling for clients and staff.
QUALIFICATIONS:

H.S. Diploma or GED.
-
Prior scheduling experience preferred.
-
Computer skills including but not limited to MS Office, MS Excel and Scheduling program.

Basic medical terminology.

Interpersonal, organizational and communication skills.

Ability to carry out directions, read and write.

Maturity and ability to deal effectively with the demands of the job.
RESPONSIBILITIES:

Understands and adheres to established policies and procedures.

Creates and maintains staff schedules.
-
Provides staffing for sick leave, vacation, long term leave.
-
Schedule patient visits according to care plans and staff availability.

Enters scheduling data, creates schedules with attention to staff call-ins and vacation time

Contacts care providers and patients regarding day-to-day changes, introductory call, discharge from hospital call and document in patient medical record

Identifies frequencies needed for each discipline, based on information received from clinicians and reports, which includes urgent requests and care plans.

Enters patient information into database.

Maintains patient database (e.g. Patient hospital admission, pending agency admissions and patient hospital discharges for SOCS and ROCS).

Performs general office duties including but not limited to word processing, photocopies, files, shreds, sort/distributes mail, provides reception and telephone services.

Upload and attach documents inside the medical record.

Work Environment:
Works indoors in office.
