Full job description

What We Offer

Licensed Clinical Social Worker

Location: Winston-Salem, North Carolina

Department: Today's Woman Health/ Wellness Center

Full Time | Monday-Friday 8am-4:30pm

Teletherapy and/or home visits

Licensed Clinical Social Worker will support Bridge to Health program which is designed to meet patients where they are, especially those whose complex medical, behavioral health and social needs cannot be fully supported in traditional outpatient settings. Many patients are managing chronic illness, the effects of trauma, or social challenges such as housing instability or poverty — factors that can place them at higher risk for poor health outcomes and lead to frequent emergency department visits or hospitalizations.

Novant Health’s Pathways to Care builds on work already underway to provide individualized support for patients with frequent hospital use, directly addressing social drivers of health and connecting patients to bridge clinics that provide comprehensive support. The Duke Endowment grant allows Novant Health to expand this model, strengthening the dedicated team already supporting high-utilizing patients through Novant Health Adult Primary Care Waughtown and Novant Health Family Medicine Shorefair.

What You'll Do

  • Work alongside Community Health Workers, MSWs, Peer Support Specialists, and other team members to address patient needs.

  • Support patients during clinic visits and coordinate care across primary care settings.

  • Follow up with patients between appointments to encourage engagement and continuity of care.

  • Assist with care transitions following hospital or emergency department visits.

  • Develop individualized treatment and care plans in collaboration with the care team.

What We're Looking For

  • Education: Graduate Degree, required. Master’s Degree in Psychology, Social Work, Counseling or related field.

  • Experience: Minimum of three years experience, required.

  • Licensure/Certification:

  • Licensed Clinical Mental Health Clinician, Licensed Clinical Social Worker

  • Additional Skills (required): Good working knowledge of computers. Ability to care for Behavioral Health Age Specific populations.

Why Choose Novant Health?

At Novant Health, we believe remarkable care starts with compassion for our patients, our communities, and each other. We value belonging, courage, personal growth, and teamwork, creating a space where everyone is respected, supported, and safe to show up as their full selves.
