Position OverviewMedical Social Workers are a trusted source of support, guidance, and advocacy for patients and families facing complex healthcare challenges. Compensation Range:
$30.58 - $45.86 per hour, depending on experience
Job Responsibilities:

Conducts psychosocial assessments of patients.
-
Develops individualized care plans based on patient needs.
-
Provides counseling and support to patients and families.
-
Coordinates with healthcare teams to ensure comprehensive patient care.
-
Advocates for patients’ needs and rights within the healthcare system.
-
Assists patients in navigating the healthcare system and accessing resources.
-
Contributes to discharge planning and continuity of care for patients.
-
Provides crisis intervention and emergency support.
-
Facilitates guardianship applications and court hearings.
-
Organizes and leads support groups for patients and families.
-
Educates patients and families about healthcare options and resources.
-
Collaborates with community organizations to connect patients with additional services.
-
Participates in multi-disciplinary team meetings to discuss patient care.
-
Assists patients with financial concerns and connects them to relevant resources.
-
Provides bereavement counseling and support.
-
Documents all patient interactions and treatment sessions in compliance with relevant regulations and organization policies.
-
Works collaboratively within a multi-disciplinary treatment team.
-
Adheres to all state and federal regulations surrounding HIPAA/Title 42 requirements and maintains strict confidentiality.

Serves as a mandated reported and follows all legal and ethical guidelines.

Participates in clinical supervision in accordance with NYSDOE regulations.

Maintains licensure throughout entirety of employment.

Master’s degree in social work

NYS Licensed Master Social Worker LMSW Limited Permit - Those who are hired with a limited permit must have their full license 1 year after obtaining their limited permit.

3 year’s post graduate experience
