Full job description

The Agency's mission is to be boldly innovative in improving the health and quality of life for South Carolinians.

This position is located in Community Long-Term Care (CLTC), Richland County. This is an in-office role and not a telecommute or remote position.

Are you the One? We are looking for a Registered Nurse, Non-Institutional who will work under general supervision of Area Administrator and Lead Team Nurse Consultant. Maintains knowledge of CLTC programs and related systems including Phoenix information system and assists other CLTC offices with essential job functions when scheduling permits. Conducts pre-admission screening for persons seeking Medicaid sponsored nursing home placement and for persons seeking admission to various Medicaid waivers and programs such Community Choices, HIV-AIDS, HASCI, Children's Personal Care, TEFRA, and PASRR. Participates in and/or conducts training on PASRR completion and assessment to staff of long-term care facilities and hospitals. Performs all and ongoing oversight and medical consultation for Ventilator Waiver and Children’s Personal Care program. Travel to location of assigned cases is necessary for this position.

  • Promotes good communication.
  • Provides program related training.
  • Performs emergency preparedness duties or activities as required.
  • Performs Other Duties as Assigned.

A Diploma in Nursing or an Associate Degree in Nursing and one (1) year of experience as a Registered Nurse. Graduation from an accredited school of nursing.

Preferred Qualifications: Bachelor's degree in Nursing.

NECESSARY SPECIAL REQUIREMENT: Current licensure as a Registered Nurse by the State Board of Nursing for South Carolina.

Additional Requirements:• Occasional overnight travel.

  • Requires holder to drive routinely.

  • Sitting or standing for long periods of time.

  • Lifting requirements: 20 lbs.

  • Valid driver's license.

  • In-office role.

  • Ability to multi-task within time frames and prioritize cases utilizing effective time management skills.

  • Effective management skills and computer proficiency.

  • Must be able to perform web-based software functions (key participant data, run reports, update documents, etc.).

  • Gerontology preparation desirable.

  • Skills in assessment, care planning and case management for long term care patients.

  • Considerable knowledge of social, mental and cultural aspects of illness.

  • Interpersonal skills for developing and maintaining participant, family and community relationships.

  • Written and verbal skills in evaluating participants' conditions and needs and interpreting and explaining CLTC program goals to participants, families and other community contacts.

  • Knowledge of HIPAA regulations.
