Salary ranges depending on home care experience:
$127,000.00 – $129,000.00 (2 – 4 years)
$129,100.00 - $131,000.00 (5 – 9 years)
$131,100.00 - $133,000.00 (10+ years)
Monday - Friday | 8:30 AM - 5:30 PM
Assigned service areas: Mainly Suffolk, Nassau if needed

What You'll Do
-
Conduct joint supervisory visits with field nurses.

Conduct in-home nursing visits for patients with complex medical needs, providing clinical assessments, treatment, and patient education.

Mentor, coach, and develop nursing and paraprofessional staff.

Audit clinical documentation for quality and regulatory compliance.
-
Ensure evidence-based nursing standards are followed.

Monitor care delivery and identify improvement opportunities.
-
Collaborate with physicians, care coordinators, and interdisciplinary teams.
-
Provide wound care, medication administration, assessments, and patient education.

Support Diagnostic & Treatment Center nursing operations as needed.
What We're Looking For
-
Active New York State RN license
-
BSN from an accredited nursing program
-
NYSED Infection Control Certification
-
Minimum 3 years of nursing experience
-
Strong assessment, communication, and leadership skills
-
Valid NY Driver's License and reliable vehicle
-
Home Care, PACE, MLTC, Managed Care, or supervisory experience preferred

CenterLight Healthcare PACE, a Program of A (https://centerlighthealthcare.org/care-services/) ll-InclusiveCare for the (https://centerlighthealthcare.org/care-services/) Elderly, serves adults 55+ throughout New York City, Westchester, Nassau, and Western Suffolk Counties. Designed for those with Medicare and/or Medicaid, the program empowers older adults to continue to live independently at home and in their communities. CenterLight’s PACE program is one of the largest nonprofit PACE programs in the country and operates 11 PACE centers (https://centerlighthealthcare.org/about/where-we-are/) and one Alternative Care Setting (ACS) where participants can receive on-site socialization, social and personal care services, therapeutic recreation (https://centerlighthealthcare.org/what-is-therapeutic-recreation/) , meals, and transportation. Our PACE centers also offer medical services, rehabilitation, prescription and over-the-counter (OTC) medications to thousands of New Yorkers by an interdisciplinary team (IDT) of doctors, nurses, physical therapists, nutritionists, recreational therapists, and other professionals who work together to provide care as unique as the diverse participants enrolled in the plan.
