Full job description
Overview
The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. In 2020, our program distinguished itself as the third in the nation to achieve Heart Failure Certification through DNV, reflecting commitment to evidence-based standards and a culture of continuous improvement. In 2022, we further demonstrated our commitment to exceptional outcomes by achieving Gold Plus Status from the American Heart Association, a designation we continue to maintain through sustained performance in quality metrics and guideline-directed care.

This role is responsible for upholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinical expertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery of exceptional outcomes for our patients.
