Full job description

Purpose:

As an employee of Trinity Health System, it is the responsibility of this and all positions within the Hospital to support the mission and role of Trinity Health System. In addition, this position will actively support the Hospital's Strategic Plan in the areas of People, Service, Quality, Finance and Growth in a team based environment. The employee also performs all job duties in compliance with laws and regulation in the observance of accrediting agency standards. The Licensed Practical Nurse promotes quality care through the administration of nursing care to designated patients under the direction and supervision of the Registered Nurse. As a member of the interdisciplinary team, he/she strives for excellence in customer service through collaboration with team members and provides a variety of direct and indirect patient care duties.

Responsibilities:

  • Contributes to the assessment of patient care through the collection and reporting of objective and subjective data. Assists in the planning of patient care through participation in interdisciplinary care. Assists with implementation of patient care activities, including assistance with patient education and medication administration to meet the health care needs of the patient. Assists with the evaluation of patient care through the prompt communication regarding changes in the healthcare needs of the patient. Maintains the competencies necessary to execute the skills necessary to fulfill the duties of the position.

  • Perform other duties as assigned.

  • Knowledge of current nursing practice, critical thinking skills, professional verbal and written communication skills, technical/computer skills, attention to detail, customer service skills, adaptability, maintain confidentiality.

Licensure, Certifications, and Clearances:

  • Graduate from an accredited School of Nursing.

  • Basic Life Support (BLS)

  • Licensed Practical Nurse (LPN). Possess current licensure in Ohio.

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
