Advanced Practice Provider - Primary Care - Frontier Internal Medicine Boulder Community Health, Longmont, CO 80026 $49.98 - $74.97 an hour - Full-Time, Days
Boulder Community Health is seeking a compassionate and collaborative Nurse Practitioner or Physician Assistant to join our Frontier Internal Medicine team. This role provides comprehensive primary care services to adult patients, including preventive care, chronic disease management, acute illness evaluation, and health promotion within a Patient-Centered Medical Home model focused on delivering high-quality, coordinated care.
As part of a multidisciplinary care team, you will partner closely with physicians, nursing staff, and clinic support teams to assess, diagnose, treat, and educate patients while promoting evidence-based practices and long-term wellness. This is an excellent opportunity for an APP who enjoys developing lasting patient relationships, managing complex medical conditions, and practicing in a collaborative outpatient internal medicine environment.

Responsibilities •Provide same-day, next-day, primary, and acute care services for patients across the lifespan. •Obtain medical histories, perform physical examinations, assess, diagnose, and develop treatment plans. •Order, interpret, and manage laboratory tests, diagnostic studies, medications, immunizations, and minor procedures within scope of practice. •Collaborate with physicians and the care team to manage acute and chronic medical conditions. •Educate patients and families on treatment plans, medications, preventive care, and available community resources. •Respond to patient calls, provide clinical consultation, and support continuity of care between visits. •Evaluate and manage urgent and emergent patient care needs. •Maintain accurate and timely clinical documentation, orders, and patient records. •Participate in daily care team huddles and collaborate with providers, medical assistants, and clinic staff to deliver evidence-based, patient-centered care. •Support quality improvement initiatives, professional development activities, and regulatory compliance requirements.
