Job Summary:

Under the direction of the Director of Centralized Services, provides ambulatory and centralized clinical pharmacy services to support medication management, medication access, transitions of care, and other patient care activities across the health system.

REQUIREMENTS

• PharmD degree or BSPharm with post-2006 experience .

• Experience with patient counseling, patient profile review, pharmacokinetic evaluation, superior time and project management, preceptorship, evidence-based literature review.

• 3 years of relevant pharmacy experience.

• Must be licensed as a Pharmacist and in good standing in Ohio or Michigan at time of hire.

• Must be a registered pharmacist, licensed in good standing in Michigan AND Ohio within 60 days of hire. Michigan licensure includes controlled substance license.

• BLS, Board Certification within 2 years of hire.

PREFERRED REQUIREMENTS

• PGY1 Residency.

• Board Certification in Pharmacotherapy or Ambulatory Care.
