Organization: SCAH-Sutter Care at Home - Yuba City Position Overview:

Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Demonstrates advanced knowledge and practice level competencies and is a recognized clinical or workflow subject matter expert. Responsible for developing key workflows and clinical protocols or guidelines for department. Serves as a resource in the education, training and mentoring of other staff and as a clinical instructor for assigned students. May coordinate staff education and student programs. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. Job Description: EDUCATION:

  • Other: Graduate of an accredited physical therapy school

CERTIFICATION & LICENSURE:

  • PT-Physical Therapist

  • BLS-Basic Life Support Healthcare Provider

DEPARTMENT REQUIRED CERTIFICATION & LICENSURE:

  • Department: Home Health, DL-Valid Drivers License Class C or Class D

  • Department: Home Health, AUTO-Automobile Insurance

TYPICAL EXPERIENCE:

  • 5 years of recent relevant experience.

SKILLS AND KNOWLEDGE:

  • Collaborates with key stakeholders in developing solutions.

  • Critical thinking, complex problems solving, decisive judgement and ability to work independently.

  • Knowledge and application of professional practice and regulatory requirements.

  • Must be able to work in demanding work environment, organize multiple priorities completing work duties within expected timelines and requirements.

  • Computer and required technology proficiency/competencies.

  • Demonstrates effective and efficient professional communication (verbal & written) skills as well as interpersonal skills, conflict resolution, teamwork/collaboration, customer service and community relations abilities.

  • Able to retain and apply new knowledge & skills.

  • Keeps abreast of professional practice development and advancement.

  • Ability to teach/educate others, public speaking.

  • Ability to develop training materials and competency assessments.

  • Develop treatment protocols.

Job Shift: Days Schedule: Full Time Shift Hours: 10 Days of the Week: Monday - Friday, Variable Weekend Requirements: Once a Month Benefits: Yes Unions: No Position Status: Non-Exempt Weekly Hours: 40 Employee Status: Regular Employees of Sutter Health and its entities may handle hazardous drugs in the course of their work, including patient care, which requires them to manage, store, prepare, receive, unpack, transport, dispose of, or administer drugs identified as hazardous or potentially hazardous by the National Institute for Occupational Safety and Health (NIOSH) and in accordance with the USP 800 guidelines.

Pay Range is $67.43 to $94.40 / hour The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
