Unit Description:

The UVMMC Home Infusion Nurse Coordinator (HINC) is responsible for assuring appropriate transition of patients to the home infusion service by creating, revising and assessing the patient plan of care relative to identified criteria. The Home Infusion Nurse Coordinator teaches patients, care givers and home health care nurses to administer infusion therapy in the home and makes recommendations regarding safety to MD and inpatient Case manager. The HINC will coordinate with health care team members to assure timely progression of patients across the continuum of care and coordinates with home health care professionals to assure all involved are prepared to participate in home infusion therapy for the patient. The HINC maintains contact with patient, care givers and Health Care Professionals through assessments via phone weekly or as needed. HINC supports quality improvement through evaluation of variances to identified protocols and maintains quality data systems, event reports and identifies and participates in quality projects and maintains inventory particular to patient needs.

  • Current RN licensure or compact licensure recognized by the State of Vermont is required.

  • Two to five years of related clinical experience such as case management, home or public health nursing experience, or a combination of education and experience from which comparable knowledge and skills are acquired.

  • Current RN licensure or compact licensure recognized by the State of Vermont required.

  • ADN required, BSN preferred

  • Appropriate experience in specific clinical area. Varies by unit.
