Employment Type: Full timeShift: Day Shift Description: Senior Coordinator, Case Management & Social Services

The Senior Coordinator provides administrative and operational support to Case Managers and Social Workers in a fast-paced acute care hospital environment. This role assists with patient discharge planning, coordination of post-acute services, documentation management, insurance and referral processes, and communication between patients, families, physicians, and community agencies. The Senior Coordinator prioritizes multiple tasks simultaneously, ensures timely follow-up on discharge needs, and supports efficient patient throughput while maintaining excellent customer service and attention to detail. Strong organizational skills, adaptability, and the ability to thrive in a demanding healthcare setting are essential.Senior Coordiantor for Case Management

The Senior Coordinator for Case Management would work under the direction of the RN Care Managers and Social Workers. This position functions with his/her peers and other care providers for problem solving and facilitating in-patient and post hospitalization care. And coordinate, oversee records and transmit information pertinent to the resource management of patients.

Job Qualifications

  • Associate's degree or High School Diploma and equivalent relevant experience required. Bachelor's degree preferred.
  • Medical assistant or Licensed Practical Nurse (LPN) highly preferred.
  • 5-7 years of customer service, Insurance knowledge medical assistance or secretarial experience preferred. Prior experience in a medical setting required
  • Ability to organize and utilize work hours effectively and with minimal supervision
  • Medical terminology is a must
  • Must be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge.

Essential Responsibilities

  • Communicate information received from payers to utilization review nurse.
  • Transmit continued stay reviews and track authorizations
  • Complete HENS/PASSR
  • Scheduling PCP/follow up appointments
  • Faxing and phoning agencies and facilities to assist with discharge referrals and continuity of care
  • Assist with delivery of charity items-clothing/DME/meal cards, etc. and complete transportation application and arrange transportation as needed for patients at discharge
  • Completing PCS forms
  • Compiling packets for transport
  • DME: Completes form, get the script, and call liaison to follow up to ensure the patient receives before leaving.
  • Sending referrals through Epic CareLink, Facilities, and IV infusion
  • Calling facilities checking on patient acceptance
  • Faxing paperwork to facilities
  • Printing documents from Epic
  • Problem Solving and Multi-tasking is a must quality
  • Build relationships with Community Partners

*This is a full-time position at Mount Carmel St. Ann's and supports the North Region to include MCNA and MCD.
