Employment Type: Full timeShift: Day Shift Description: Full-Time LPN Hospice Nursing Home Team – Capital Region Schedule : Monday through Friday - 8:00 a.m. to 4:00 p.m. No on-call or mandated overtime! The Community Hospice is seeking a Full-Time Licensed Practical Nurse (LPN) to join our Nursing Homes team. Join a compassionate team dedicated to making a difference in the lives of patients and families! This rewarding role is ideal for an LPN who values teamwork, relationship-building, and delivering compassionate end-of-life care.

What you will do:

  • Provide hospice care for patients residing in nursing homes throughout the Capital Region.
  • As part of our interdisciplinary hospice team, you will collaborate closely with Registered Nurses, Hospice Aides, Physicians, Social Workers, Chaplains, Complementary Therapists, and Volunteers to provide exceptional care and support to patients and their loved ones.

Qualifications Required:

  • Current New York State Licensed Practical Nurse (LPN) license.
  • 1 Year LPN experience
  • 1 or 2 years of homecare and/or Hospice experience preferred
  • Valid driver's license and reliable transportation.
  • Strong communication, documentation, and organizational skills.

Pay Range: $24.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
