About the Role: ComForCare Home Health Care - West Bloomfield is looking for a compassionate and skilled LPN Nurse to join our dedicated team in West Bloomfield Township, MI. Responsibilities:

  • Provide direct patient care in home health settings under the supervision of an RN or physician

  • Administer medications, treatments, and wound care as prescribed

  • Monitor and document patient health status, vital signs, and progress

  • Develop and follow individualized patient care plans

  • Communicate effectively with patients, families, and the broader care team

  • Educate patients and caregivers on health management and best practices

  • Ensure compliance with state regulations and company policies in home health care

Requirements:

  • Active LPN license in the state of Michigan

  • Previous home health, long-term care, or clinical nursing experience preferred

  • Strong knowledge of medication administration and wound care protocols

  • Excellent communication, documentation, and organizational skills

  • Reliable transportation and valid driver's license

  • Compassionate, patient-focused demeanor with a commitment to quality care

  • CPR/BLS certification required or ability to obtain upon hire
