Status: Part-time, Hourly

Shift Hours: 8:00 am- 4:30 pm

Weekend Work: Yes

On Call: No
The Discharge Planner I responsibilities include:
-
Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.

Possesses a clear understanding of discharge and transition planning and linkage with community resources.
-
Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.

Provides support as active member of the treatment team.

Will perform other duties as assigned by supervisor.
Travel may be required.
Required Certification:
- BLS (Basic Life Support)
Required Experience:
-
Behavioral Health - 1 year
-
Child and Adolescent - 2 years (SIPP Only)
-
Acute Care - Preferred 1 year
Required Education:
-
Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing

Master's in human services - Preferred
