Empowering Lives: Mosaic at Home Coordinator

Location: Central Iowa-Des Moines Metro area

Status: Salaried, Days with OnCall to support case load (can be outside of business hours), Travel for home visits (can be up to 2 hours from Des Moines) agency vehicle or mileage reimbursement.

What You’ll Actually Do (The Good Stuff):

  • Build Epic Connections: You’ll be the go-to person, developing rock-solid relationships with the individuals we support, their families, host home providers, and direct care workers. Exceptional customer service is just how you roll.

  • Be a Dream Champion: Help individuals express their goals, dreams, and needs. You're there to make sure their voices are not just heard, but loudly respected.

  • Relationship Builder: You’ll monitor the relationships between individuals and their home providers, ensuring the pairing is a perfect, positive, and supportive match.

  • Architect Personalized Plans: Collaborate with our service planning team to create Individual Support Plans. You’ll weave together a person's unique wishes with the necessary policy and regulatory requirements.

  • Keep it Real in the Field: Oversee service delivery and contract compliance by hanging out where the magic happens. You’ll conduct regular home and service environment visits.

  • Be the Safety Net: Provide on-call support to our wonderful providers and direct care workers when they need a helping hand or a quick problem solved.

Requirements

What You’ll Bring to the Table:

  • The Degrees & Deeds: A Bachelor’s degree in human services or related field is preferred, along with a minimum of two years of related work experience.

  • The Bonus Points: Previous experience working with people with intellectual disabilities is a huge plus!

  • The Credentials: A valid driver's license in good standing (you've gotta do those home visits!).

  • The Superpowers: Top-notch communication skills, a knack for relationship-building, excellent problem-solving abilities, and computer proficiency.

  • The Know-How: Experience in behavior management and a good grasp of HCBS industry regulations.
