Day shift (9a-9p), 12-hour shifts, rotating weekends and holidays

Pay Range: $38.20 - $57.30

Registered Nurse (RN) Advocate Health | Mobile Hospital at Home Program 📍 Park Ridge, IL (Lutheran General Hospital)

Be the Nurse Who Redefines Care Step outside the traditional hospital walls and deliver hospital-level care directly in patients’ homes. This innovative role blends the autonomy of delivering care in the home setting with the complexity of acute inpatient nursing, supported by a collaborative interdisciplinary team.

  • Manageable Caseload: 3–4 patient assignments, which may include admits/discharges, per 12-hour shift

  • Flexible Schedule: Includes alternating weekends

What You’ll Do

  • Deliver acute, hospital-level care in patients’ homes

  • Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)

  • Monitor patient conditions and recognize changes requiring escalation

  • Collaborate with a virtual and in-person interdisciplinary team

  • Use evidence-based practice to ensure safe, patient-centered care

  • Coordinate discharge planning and ongoing care needs

  • Promote a culture of safety and high-quality outcomes

What We’re Looking For

Required:

  • Active RN license

  • BLS certification (AHA)

  • Valid driver’s license with a clean driving record

  • 2+ years of recent acute inpatient care or ED experience preferred

Skills:

  • Strong IV, phlebotomy, and clinical assessment skills

  • Ability to work independently in home settings

  • Excellent communication and critical thinking

  • Comfortable with technology and remote monitoring tools

Work Environment

  • Travel within designated service area

  • Provide care in private home settings

  • Physical activity required (lifting, bending, standing, etc.)

  • Exposure to clinical and environmental variables typical in-home care
