Our Greenfield Hospice office has expanded to the Council Bluffs, Iowa area and we’re currently looking for an experienced Hospice Clinical Manager - RN to support and lead our Council Bluffs clinical team. Care Initiatives, created in 1989, is the largest non-profit single operator of quality nursing homes in Iowa. With over 40 skilled nursing communities throughout Iowa and nine (9) hospice locations — Des Moines, Cedar Rapids, Greenfield, Albia, Muscatine, Waterloo, Manchester, Sioux City, Omaha, Nebraska. In this role you will lead and support the clinical team within the Council Bluffs, Iowa territory. There will be no regular travel to the Greenfield, Iowa territory.

Key Responsibilities of a Hospice Clinical Manager:

  • Oversees all visits from referral-to-admission and through death processes.

  • Ensures quality care, documentation and collaboration are within guidelines.

  • Supports direct care coordination, ensures hospice eligibility, and oversees recordkeeping functions.

  • Provides orientation, education, training and/or competencies, as needed.

  • Maintain and implement operational policies and procedures to meet patient and family needs in compliance with federal, state, and local requirements guidelines and Care Initiatives Hospice policies and procedures.

  • Support and facilitate educational directives of the Administrator and/or corporate leadership.

What it takes to be a Clinical Manager with Care Initiatives Hospice:

  • Active RN license (or ability to activate) in the state(s) in which you operate.

  • Active CPR certification.

  • Hospice or palliative care experience.

  • At least one year in a leadership or charge nurse position or relative experience.

  • Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.

  • Home access to the internet.

  • Hospice and Palliative Registered Nurse Certification (CHPN) preferred, or ability to obtain within 24 months of employment.
