TRANSPORTATION / PATIENT SCHEDULER

We are seeking a dependable and organized Transportation / Patient Scheduler to support our nursing home residents and transportation needs.

Responsibilities:

  • Schedule and coordinate resident transportation for medical appointments, therapy, dialysis, and other off-site services

  • Coordinate appointment times with residents, families, transportation providers, and medical offices

  • Maintain an accurate transportation and appointment calendar

  • Arrange transportation based on resident needs and appointment schedules

  • Communicate appointment and transportation details to nursing and other departments

  • Prepare residents and required paperwork for scheduled appointments

  • Confirm transportation arrangements and follow up on changes or cancellations

  • Maintain accurate records and assist with other administrative duties as needed

  • Provide excellent customer service to residents, families, and healthcare providers

Qualifications:

  • Previous healthcare, nursing home, scheduling, transportation, or administrative experience preferred

  • Strong organizational and communication skills

  • Ability to multitask and manage changing schedules

  • Professional and compassionate when working with residents

  • Computer skills and attention to detail

  • Reliable, dependable, and able to work as part of a team
