As an RN for the Rapid Response Program, you will provide professional nursing care across a variety of ambulatory care settings while supporting fellow caregivers in delivering exceptional patient experiences. In this role, you will work alongside the ER Rapid Response physician to respond to acutely ill patients and accompany them to local hospitals as directed, serving as a key liaison between the patient, physician, and receiving hospital to ensure timely care and coordination of transfers to Weston Hospital. You will also support other departments with prior authorizations, telephone triage, and additional needs as they arise. This position interacts directly with patients, families, and visitors, offering the opportunity to enhance their experience and supply healthcare information as needed.

A caregiver in this role works day/evening/night/weekend shifts from 8:00 a.m. – 12:00 p.m. or 1:00 p.m. – 5:00 p.m.

A caregiver who excels in this role will:

  • Assess health status by completing a nursing assessment.

  • Perform telephone triage.

  • Notify physicians/LIPs of abnormal findings.

  • Establish a care plan in collaboration with the patient, family, and health care team.

  • Collaborate with care coordinators to ensure smooth and effective transitions of care.

  • Assist with specialized diagnostic and therapeutic procedures.

  • Administer medications and treatments as ordered by physician/LIP and monitor patients for response.

  • Initiate CPR and other emergency measures.

  • Provide health education to patients and families.

  • Develop, evaluate, adapt and document education to learning needs.

  • Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience and patient safety.

  • Maintain professional growth and development in the nursing field and identify annual core competencies.

Minimum qualifications for the ideal future caregiver include:

  • Graduate from an accredited School of Nursing (ADN or BSN program)

  • Current state licensure as a Registered Nurse (RN)

  • Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross

  • *In areas performing moderate sedation procedures and/or cardiac stress testing, must have current Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) or ALS through American Red Cross Certification within six months of hire

  • One year of recent experience as an RN

  • OR LPN, Medical Assistant and/or other unlicensed clinical healthcare experience if applicant is a current Cleveland Clinic caregiver in an ambulatory setting transferring into an RN role but remaining in the same current ambulatory Cleveland Clinic setting

  • OR a current Cleveland Clinic caregiver in a clinical role within the same specialty area transferring into an ambulatory area

Preferred qualifications for the ideal future caregiver include:

  • Bachelor’s of Science in Nursing (BSN) within three years

  • Two years of experience

  • Outpatient experience

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.

  • Requires corrected hearing and vision to normal range.

  • May requires some exposure to communicable diseases or bodily fluids.

  • Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects.

  • Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.

  • Follows Standard Precautions using personal protective equipment as required for procedures.
