City/State Suffolk, VA Work Shift First (Days) Overview: THIS IS A PRN/FLEXI POSITION 2 shifts per month Our highly skilled team of clinicians with Sentara Home Health Services partners with individual physicians to coordinate the multiple therapies and services each patient may require – from skilled nursing care and infusion therapy to respiratory care and medical equipment. Sentara Home Health Services is accredited by the Accreditation Commission for Health Care and belongs to a number of local, state and national organizations. Sentara Home Health Services has been ranked in the Top 10 Home Care Agencies in America by Home Care Magazine over the past several years, and has won the Best-Practice Award for Asthma Management (American Association of Health Plans). We are Medicare-Medicaid Certified for home health and Medicare-Medicaid & Tricare hospice certified
- Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury.
- Provides age appropriate education for clients and caregivers.
- Supervise and assume responsibility for care provided by identified team members.
- Master's or Doctorate Level Degree preferred. 1 year experience required for Resource Pool applicants.
- For Hampton Roads Outpatient facilities only - Must obtain Direct Access certification within one year of hire for those with a BS or MS degree

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $103,376.00 - $149,115.20 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
