St. Joseph of the Pines is seeking a full-time Registered Nurse (RN) Manager/Clinical Care Coordinator to support the delivery of high-quality, resident-centered care within our Skilled Nursing Facility. Reporting to the Director of Nursing, this role coordinates clinical services for an assigned resident population, oversees care planning, and collaborates with residents, families, and the interdisciplinary team to achieve positive clinical outcomes.
Responsibilities

Complete resident assessments and develop individualized care plans.

Coordinate admissions, discharges, and ongoing clinical care.

Monitor resident outcomes and update care plans as needed.

Administer medications and treatments in accordance with established policies.

Provide clinical leadership, delegation, and support to nursing staff.

Collaborate with residents, families, providers, and care teams to ensure quality care and resident satisfaction.
Qualifications
-
Graduate of an accredited School of Nursing.

ADN or BSN required.

Current Registered Nurse License in North Carolina.

Minimum one year of recent long-term care or skilled nursing experience.

Strong clinical assessment, communication, and leadership skills.
