DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS The Houston Health Department seeks a detail-oriented Licensed Vocational Nurse (LVN) to support the Dawn Program within the Office of Chronic Disease. The position will provide clinical and health education support, conduct biometric screenings, assist with patient/client engagement and follow-up, perform chart reviews and documentation, support billing and reimbursement activities, and assist with program data collection and reporting. The position may also perform other HHD clinic and Office of Chronic Disease responsibilities, as needed.

Duties include the following:

• Interview clients and obtain, document, and verify medical and health histories using designated electronic health records, databases, and other approved information systems.

• Perform biometric and health screenings, including blood pressure, pulse, temperature, A1C, lipid panels, weight, height, BMI, and other screenings within the scope of practice and program protocols.

• Provide clinical support and services within the LVN scope of practice, including administering medications, vaccines, and other treatments in accordance with approved standing delegation orders, policies, and procedures.

• Provide client follow-up and care coordination to support engagement, adherence to recommended services, and achievement of individualized health goals. Escalate clinical concerns to the appropriate nurse or provider.

• Provide health education, health coaching, and chronic disease self-management support related to diabetes, hypertension, cardiovascular health, nutrition, physical activity, and other chronic disease prevention and management topics.

• Assist with the delivery of evidence-based chronic disease interventions and health promotion activities, including individual and group education sessions, outreach activities, and other Dawn Program services.

• Perform chart reviews, record reviews, and clinical documentation audits to assess completeness, accuracy, quality, and compliance with program, HHD, and applicable billing requirements.

• Collect, enter, maintain, and validate programmatic and clinical data in designated electronic systems and databases to support program monitoring, quality improvement, billing and reimbursement, and reporting requirements.

• May assist with preparation of routine, grant-related, accreditation, performance, and management reports, including compiling data, calculating performance measures, tracking client services and outcomes, and ensuring timely and accurate submission of required reports.

• Assist with maintaining inventory of clinical supplies, equipment, and medications, including monitoring supplies, identifying shortages, and maintaining a clean, safe, and organized clinical environment.

• Support positive client outcomes and quality service delivery across Dawn Program and other Office of Chronic Disease clinical and health promotion activities.

• Perform other duties as directed.

WORKING CONDITIONS The position routinely requires lifting of moderately heavy items, such as record boxes (up to 40 pounds) and/or very long periods of walking on rough surfaces on a routine basis.

MINIMUM REQUIREMENTS

MINIMUM EDUCATION Requires a high school diploma or GED certificate and graduation from a School of Vocational or Practical Nursing.

MINIMUM EXPERIENCE: Six (6) months in a healthcare environment is required. LICENSE / CERTIFICATION: Must be licensed or eligible to practice as a Vocational Nurse in the State of Texas. CPR certification required. License and CPR certification must be maintained and current for continued employment. Valid Texas Driver’s License and compliance with the City of Houston's policy on driving (AP 2-2).

PREFERENCES

Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.

• Directly related professional experience or training in Diabetes or geriatric care preferred.
• Preference for applicants that are bi-lingual in English, Spanish, and Vietnamese based on community needs.
• Experience using Microsoft Office products including Word, Excel, PowerPoint, Outlook.
• Experience delivering one-on-one patient education and group presentations preferred.

GENERAL INFORMATION

SELECTION/SKILLS TESTS REQUIRED None. However, the Department may administer a skills assessment test.

SALARY INFORMATION GENERAL FUNDED POSITION Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
