Job Title: Home Care Scheduler / Client Care Coordinator Location: Santa Monica, CA (On-site, 14th Street) Hours: Monday – Friday, 8:00 AM – 5:00 PM (Full-Time) Pay Range: $23.00- $28.00 per hour (Based on experience)

Job Summary: As a Client Care Coordinator/Scheduler, you will act as the primary liaison between our clients, prospective families, and caregivers. Prior staff scheduling experience is strictly required for this position. Your main responsibility is to ensure all client shifts are accurately scheduled and staffed using WellSky software. You will also manage client care plans through routine assessments, and actively support HR by onboarding, coaching, and managing employee relations with our caregiving staff.
Key Responsibilities:
• Scheduling: Match caregivers to client schedules in WellSky; input new client data and manage daily schedule changes.
• Client Assessments & Care Plans: Conduct intake assessments for prospective clients, perform care reassessments for current clients, and update care plans to ensure service accuracy.
• Caregiver Relations & Coaching: Serve as the point of contact for care staff; provide guidance, address disciplinary matters, handle employee relations issues, and coach caregivers to ensure high-quality care.
• Communication: Manage incoming phone inquiries professionally and maintain ongoing communication with families to monitor satisfaction.
• Payroll & Billing: Verify caregiver clock-ins/outs and update computer records for accurate billing and payroll processing.
• HR Support: Assist with caregiver recruitment, onboarding, orientations, and keeping personnel files compliant.
• Emergency Coverage: Step in as a backup caregiver to assist clients during staffing emergencies or call-offs (up to 30% of the time).
Qualifications:
• Experience (Required): Must have 6+ months of staff scheduling experience within a Home Care Organization, Long-Term Care (LTC) facility, or similar environment.
• Experience (General): 1+ years of general office/customer service experience and 6+ months of hands-on caregiving experience (CNA license preferred)
• Software: Familiarity with WellSky or similar scheduling software is required; intermediate to advanced Microsoft Office and internet skills.
• Education: High School Diploma/GED required (Bachelor’s preferred).
• Credentials: Valid driver’s license with an insured vehicle, ability to lift 50 lbs, a current TB test, and CPR certification (or willingness to get it within 1 week).
• Licensing: Registered with the CA Department of Social Services as a Home Care Aide (can be obtained post-hire).
