Care coordinators work collaboratively to facilitate the planning and organizing of critical activities for our patients. Reporting to the Manager/Supervisor, this role will coordinate with providers and Integrated Care Nurses to provide ongoing support and communication to Chronic Kidney Disease (CKD) and/or End Stage Renal Disease (ESRD) patients. The Care Coordinator, Intermediate acts as a single point of contact to coordinate resources for DaVita IKC patients. They collaborate with RN case managers and Nurse Practitioners to make sure our patients receive the highest quality care. Working in clinic/partner practices 3-4 days per week, working remotely the remaining day(s).

Coordinates care for our patients by ensuring they can access eligible resources/programs and assisting with referral process and provider appointments.

Performs outbound calls and responds to inbound calls from patients, providers, and Village resources. Under guidance of Manager, completes non-clinical pathways & screenings; escalates to Integrated Care Nurse as needed.20% Assists clinical team with moving patient through the continuum of care by completing tasks as directed.

Helps identify eligible resources for members and works with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in eligible programs.

Other duties as assigned.

2-4 years experience High school diploma

Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook Required

At least 2 years of experience in a professional office environment Required Prior experience working in a medical office environment. Preferred Prior experience coordinating care for a patient (e.g. referrals, ACO care management, etc.) Preferred

Prior experience with insurance payors and/or Medicare Preferred Prior experience working in dialysis clinic and/or with renal patients Preferred

The Wage Range for the role is $18.00 - $22.50 per hour.

If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.

New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour
