Job Description:

Mileage reimbursement provided: $0.76 per mile

Schedule: Monday - Friday, 8am - 5pm; weekend on-call rotation

Company provided phone and laptop/iPad

• Develops professional and clinical skills through attendance at staff development meetings, educational activities, and professional affiliations.

• Admits new patients according to departmental policy and performs comprehensive initial evaluations for those requiring physical therapy.

• Conducts home safety evaluations and communicates specific needs to the physician and appropriate team members.

• Establishes a plan of care with goals based on the patient’s current level of functioning and potential for improvement, in conjunction with the patient, family, caregiver, and physician’s orders. Implements the physical therapy assessment and care plan, continuously evaluating the patient’s response and modifying as needed. Reports pertinent observations to the attending physician and adjusts the patient care plan as directed.

• Instructs patients and families in the physical therapy program, including the use of prosthetic, orthotic, and assistive devices.

• Participates in the team process, through cooperative efforts with team members, in developing the patient care plan and attending team conferences.

• Supervises and plans home health aide activities in the absence of other healthcare professionals, according to individual patient care requirements and department policy.

• Demonstrates effective communication skills with all team members, patients, and families.

• Maintains clinical record information in accordance with established standards and within established time frames.

• Participates in the review process, agency performance improvement program, orientation, in-service, and staff meetings as scheduled.

• Displays flexibility in accepting assignment changes and patient care responsibilities, including participation in weekend coverage and call rotation.

• Other duties as assigned.

Pay Range:

$33.82 - $62.90
