J ob Summary
The Community Health Worker (CHW) is a frontline member of the mobile clinic team, responsible for patient engagement, outreach, navigation of social services, and supporting clinical staff in delivering high-quality care. This role helps bridge gaps in access by meeting patients where they are—streets, shelters, encampments, schools, and community centers. The CHW builds trusting relationships, provides culturally responsive support, and ensures patients receive the resources they need to improve their health and stability. Must be willing and able to travel to multiple sites. Comfortable working in outdoor, mobile, and unpredictable environments.
Job Requirements
M inimum Education Requirement:
High school diploma or equivalent required. Additional education in health or social services preferred. Bilingual in Spanish preferred.
Minimum Experience Requirement:
Experience working with underserved or vulnerable populations (homelessness, low-income communities, immigrants, etc.). Strong communication skills and ability to build trust with diverse groups. Strong organizational skills and comfort with basic technology/data entry. Knowledge of local community resources and social service systems preferred.

Pay Range:
$23.00 - $34.50
